Back2Basics works with Carelon Behavioral Health members — young adult men in Flagstaff, Arizona. We check exactly what your plan covers, and what it would cost you, before you commit to anything. Your plan may cover residential treatment, dual diagnosis care and sober living — in a program built around the outdoors rather than a clinic hallway.

Carelon Behavioral Health is a network, not an insurer. It does not decide whether your treatment is covered — your underlying plan does. Employers, self-funded plans and third-party administrators rent access to the Carelon Behavioral Health network to get negotiated rates from providers.
So there are two questions, and they have different answers. Is residential treatment covered? is answered by your plan, which for most commercial and employer coverage must include substance use disorder treatment on terms comparable to medical care under federal parity law. What does it cost? is where Carelon Behavioral Health comes in, through the rate your plan pays. We check both — your card often carries two logos, and we need each of them.

If your Carelon Behavioral Health coverage is an AHCCCS (Arizona Medicaid) or Medicare product rather than a commercial plan, Back2Basics is not able to bill it. We are not an AHCCCS-contracted provider, and that is true no matter which carrier administers the plan.
If that is your coverage, please still call. We will point you toward AHCCCS-contracted residential programs in Arizona, and we will talk honestly about private-pay rates and our scholarship program. Being told the truth on day one is worth more than being told what you want to hear.
Back2Basics is a residential and extended-care program. We are not a detox facility and we do not run PHP or IOP — if you need medical detox first, we will help you coordinate it and hold your place.

“Covered” does not mean free, and any program that implies otherwise is selling you something. Here is how the money actually works, in plain terms:
Your deductible is what you pay before the plan starts contributing. If you are early in the plan year and it is untouched, your first bill carries more of it.
Your coinsurance is the percentage you keep paying after the deductible is met — often 10% to 30% in-network.
Your out-of-pocket maximum is the ceiling. Once you reach it, Carelon Behavioral Health covers 100% of covered services for the rest of the plan year. For a stay of 60 to 90 days, many families hit this ceiling — which is why the total cost of a long stay is often much closer to the cost of a short one than people expect.
The single biggest lever is whether your plan treats us as in-network or out-of-network. Out-of-network care is billed against a separate, higher deductible and reimbursed at a lower rate — sometimes still very workable, sometimes not. We tell you which schedule applies to you up front, in writing, rather than after admission.

Back2Basics is based in Flagstaff, at 7,000 feet in the ponderosa pine country of northern Arizona — with the San Francisco Peaks, the Grand Canyon and Sedona all inside a short drive. That geography is not scenery. It is the reason the program works the way it does: mountain biking, climbing, snowboarding, rafting and backcountry trips are part of the clinical week, not a weekend reward.

Yes. Plans carrying Carelon Behavioral Health cover medically necessary residential treatment for substance use disorder, and federal parity law requires that coverage to be comparable to what your plan offers for medical care. We verify whether your specific Carelon Behavioral Health plan treats Back2Basics as in-network or out-of-network, and exactly what that means for your share, before you commit to anything. What you actually pay depends on your deductible, coinsurance and out-of-pocket maximum — we walk you through those numbers before you commit.
There is no fixed number. Coverage is authorized in increments — often 7 to 14 days at a time — and extended based on clinical progress and continued medical necessity. Our clinical team handles those reviews directly with the insurer throughout your stay, so you are not the one making those calls. Most young men in our program stay 60 to 90 days.
Usually, yes. Most Carelon Behavioral Health plans require prior authorization for residential treatment. We obtain it for you as part of admissions — typically the same day we receive your insurance information, and often within a few hours.
That depends on where you are in your plan year. If your deductible is already met, your cost may be limited to coinsurance until you hit your out-of-pocket maximum. If it is January and nothing has been applied, your share will be higher. We give you a written estimate before admission rather than a surprise bill after — and if the number is out of reach, ask about our scholarship program.
